Application Forms PSC form3 (revised 2008) can be obtained from the office of the secretary district service commission Amudat or any district service commission offices country wide or from public service commission Kampala. Application forms in triplicate with three recent passport size photographs and photocopies of certified relevant documents should be submitted to the secretary service commission, P.O Box 2622. Mbale NOT later than Wednesday the 10th April 2019, clearly indicating the Reference Number of the post applied for.
