ORGANIZATION BACKGROUND
Friends of Christ Revival Ministries Uganda (FOC-REV) is a non-governmental, nonprofit making, non —political, inter-denominational charitable organization that was established in 1999 by a group of volunteers as a response to a crisis caused by HIV &AlDS and poverty in rural communities in Uganda. FOCREV Uganda exists to work with the poor and marginalized rural people to improve their livelihood and increase self—reliance through education support, health services advocacy, food security and networking.

FOCREV has entered into partnership with Catholic Relief Services to implement Sustainable Out Come for Children and Youth interventions in Hoima and Luwero Districts Over a period of two Years and would like to recruit competent and qualified people to till the following vacant positions.

Job Title: Linkages and Referral Coordinator
Reporting: To Project Manager
Duty Station: Luwero District

Main Purpose of the Job
The Referrals and Linkages ‘Facilitator will play a pivotal role in Developing and ensuring functional bi-direction ureteral systems Between communities, clinical services and social services Providers.

Key Responsibilities of the Linkages and Referral Coordinator.

• Lead the establishment and or strengthening of community OVC referral and senrice coordination networks in the target communities.
  
• Continuously track and monitor success of the referral systems, and provide regular technical support to community and facility based referral facilitators or volunteers.
  
• Work with other regional or District based USAID funded HlV/AIDS partners to support and facilitate beneficiary HIV Testing and Counseling, linkage to care and Plan and conduct quarterly Sub county and district level referral reflection and service provider coordination meetings with other key bidirectional referrals stakeholders.
  
• Coordinate with Social Workers and other USAID lPs to plan and conduct targeted OVC service camps in hard to reach program communities
  
• Generate a referral progress report on a monthly basis and conduct monthly case conferences with community volunteers
  
• Utilize program data to track beneficiary linkage into HIV/AlDS care and adherence
  

Required Qualification and Experience

• Minimum of Bachelor's degree in Social Work, public health, social sciences or related qualification
  
• Post graduate training in child protection methodologies and best practices
  
• Demonstrated 2 years’ experience implementing linkage and Referral initiatives with government and/or USAID funded projects
  
• Experience/knowledge of case management identification and assessments, planning, referral and support services, monitoring, review and closure tools
  
• Excellent interpersonal and communication skills
  
• Ability to work independently, multi-task and take initiative.
  
• Strong leadership capabilities and demonstrated potential for learning new skills
  
• Familiarity with OVC and HIV-related programming
  
• Valid Motorcycle riding permit
