JOB DETAILS:
Main Objective:
The main objective of the consultancy is to develop a comprehensive community health investment case that takes into consideration the national and global developments in community health. The Investment Case will provide an in-depth analysis of the funding flows, financing needs and gaps in the Uganda Community Health Program. Additionally, it will explore in-country plans and options for scaling up efforts to meet the program needs in a more sustainable way. The consultancy team will develop a roadmap towards financial sustainability of the program interventions and generate a strategic report that will cover the following:
I. A financial-focused analysis of health system bottleneck.
II. Strategies to address bottlenecks and develop scenarios for the community health program in Uganda.
III. Estimated costs of implementing identified strategies in different scenarios.
IV. Investment returns of implementing the identified strategies and Scenarios.
V. A fiscal space analysis and financing opportunities, and a series of recommendations for investing in community health system strengthening.

Specific objectives:
The team of consultants, preferably two; will outline an appropriate methodology or approach to achieve the deliverables highlighted above that include a situational analysis, current financing commitments versus projected gaps and scenarios for return on investment in the case of Uganda.
• Summarize aggregate country-level data related to current financing and commitments to Community health program including community health service delivery.
• Identify strategies to fill the financing gaps for community health interventions and provide different scenarios / options for an effective program.
• Determine affordable remuneration to Community Health worker to deliver integrated community health services using an appropriate method e.g., optimization methods.
• Develop options for increased national investment in community health program and share with the GoU/MoH on the feasibility of the future investment in community health workers.
• Develop the costing for community health interventions using an appropriate costing methodology.
• Generate an implementation plan or road map of Community Health investment case towards financial sustainability including a monitoring and evaluation framework.
• Provide details on benefits and savings in investing in community health program in different areas.

Work Assignment:
The consultant team will be expected to:
• Undertake a literature and desk review of available documents on national data (reports from Ministry of Health (MoH), Ministry of Finance and Economic Planning (MoFPED), National Planning Authority, Ministry of Gender Labor and Social Development, Ministry of Public Service), policies and strategies related to community health financing and services delivery, community health evaluation/assessment reports, Community Health Reform, Off-budget resource tracking reports for community health, National Health Accounts (NHA) reports, etc.
• Analyse community health landscape in Uganda, including major policy shifts (global and national) and their implications and primary qualitative data collection through key stakeholders’ interviews.
Use of optimization methods in community health care to determine an affordable remuneration package for CHWs to deliver Community health integrated package.
• An in-depth analysis of the current financing situation of community health services through analysis of in-country funding flows for community health disaggregated into: The overall need, gap between overall need and current availability (Define the current resource gaps for community health services and for ten years);
• An in-depth analysis of the cost required to deliver an integrated package of community health interventions to community by CHWs in rural and urban area (the cost includes training materials, training, supervision, incentives etc.)
• Determine projected financial scenarios at the national level based on national targets.
• Outline potential innovative strategies and options to meet needs, to increase and sustain funding for community health in country.
• Clarify the economic benefits of investing in community health in mid and long term; estimate the return on investment for the specific case of Rwanda (including with an analysis of innovative financing approaches that would facilitate tapping into domestic/household finance as contribution to meeting overall needs).
• Develop an implementation plan or road map for financing Community health program at the national level taking into account the fiscal space in the country, identifying who, when, how and what affects the decision making of domestic resources allocation for Community health services and commodities and document its feasibility towards financial sustainability along with its monitoring and evaluation framework.
• Draft a comprehensive Community health investment case with its different scenarios of finance/cost required.
• Lead the process of validation of the draft comprehensive Community health investment case with its different scenarios of finance/cost in workshops or meetings to get stakeholders’ inputs.
• Submit the final agreed design, edited and layout of the community health investment case to the Ministry of Health, Department of community health and USAID/UHSS.

Deliverables

The assignment is divided into three main deliverables that include:

Deliverable
1: An inception report that outlines the key findings from the literature/desk review, detailed methodology for developing the investment case and Gantt chart with timeline to complete the assignment and main discussion points raised and agreed at the stakeholder’s consultation meeting and cleared by Community Health technical working groups (TWGs).

Deliverable
II: First draft of the community investment case of different scenarios informed by recent evidence, identified gaps and key priority areas including proposed strategies related to planning/design, implementation, monitoring, and evaluation of community health program packages with respect to accessibility to services, quality, and funding sustainability to be shared, presented, and validated by Community Health technical working groups (TWGs).

Deliverable
III: Submission of a validated Community Health investment case for Uganda, final edited and well-designed print ready document.

All the deliverables will however need to be validated by USAID/UHSS, Community Health Department, community health TWG, SMC TWG among others. The write up of all outputs above should be in a clear language so that they can be easily understood, avoiding long sentences, jargon, abbreviations, and technical terms to the extent possible, and should as necessarily define the terms used.

Desired competencies, technical background, and experience

Mandatory Qualifications:
• At least one consultant has a minimum of 1 year leading the development of health-related investment cases including primary health care or community health programme investment case, health sector strategic plans, health financing strategies/plans and M&E plan in the region as well as in other developing countries.
• The lead consultant should hold at least a master’s degree in health economics, public health, or Health financing with an extensive progressive professional design of health strategy/plans and financing reform, with a track record of experience in leading the development of Community health programs, investment case in primary health care, resource mobilization, health worker motivation and financing, development of community health investment case/strategy or policy among others.
• The lead consultant should have at least 8 years’ minimum experience in coordinating and participating in similar assignments with a clear understanding of Community Health Programming in Uganda.
• The other member of the consultant team should hold at least a master’s degree in public health, medicine, and related field with an experience in epidemiology and Burden of Diseases modelling, design of public health strategies, policies, and related documents at least 8 years in similar work is required.

Desired qualifications:
• Experience with developing community health program investment case, health sector strategy plan, costing and M&E plan in developing countries.
• Capacity of the consultant team to carry out the scope of work with similar complexity.
• Demonstrable experience producing similar documents.
• Data collection expertise including interview and focus group discussion with at least five years’ experience.
• Demonstrated successful experience in evidence-based publications, cost effective analysis, financial sustainability plans, policy briefs, strategies, guidelines, reports; etc.
• Demonstrated experience with analysis of complex data using advanced software and costing.
• Fluency in English with excellent writing, analytical, conceptual and communication skills.
• Knowledge of Uganda health system is an asset.
• Evaluation and Selection Criteria of the consultancy Institution
• Evaluation process and methods
• Separate Technical and Financial proposals should be submitted in sealed envelopes. A two-stage procedure shall be utilized in evaluating proposals, with evaluation of the technical proposal being completed prior to any financial proposal being compared. A 80/20 assessment approach for the technical and financial proposal respectively will be adopted. Bidders must score a minimum of 70 points on the Technical Proposal and meet the Mandatory Qualifications to be considered technically compliant for the component, and for the Financial Proposals to be opened.

The criterion to be used for the evaluation of proposals is outlined in the below:

Technical proposal:
• Narrative proposal of maximum 10 - 15 pages including at least: individual consultants’ expertise and experience, proposed approach and methodology including proposed timeframe for each deliverable, Proposed team structure/responsibilities and expertise/experience.
• Annexes: Workplan of scheduled activities, copies of two reports of previous community investment case/other investment cases in the health sector and/or community health programme costing, or investment or strategy developed by the firm and CVs of all team members can be included as Annexes, in addition to the maximum 5-10 pages of narrative.

Financial proposal:
All bidders must submit a stand-alone budget for: a) Staffing by team member, b) Direct Costs, and c) Indirect Costs.
