Job Summary: The Community & Psychosocial HIV/AlDS Services (CPS) Advisor is responsible for all project activities that promote community based services and facility

  • Community linkages along the continuum of care at HGF.

Key Responsibilities:

  • Takes overall responsibility and technical leadership for the community based treatment & support teams, adherence systems at ACT supported HCF.
  • Develop and distribute appropriate job aides/SOPs an education materials for treatment support staff an counseling services at HCF. /
  • Document and share key lessons learnt from various treatment & support models, client tracking, adherence systems and their effectiveness.
  • Network with other organizations involved in community based client support like adherence to ART and promote linkages between HCF and organizations providing HlV support services in their community.
  • Support HCF in developing evidence-based clinical and community protocols and tools for community based interventions to support HIV patients in care and treatment in line with the MOH community strategy.
  • Develop and disseminate counseling guidelines on treatment literacy, adherence assessment and support to HCF in line with national standards.
  • Coordinate training of expert clients for adherence
  • support and promote positive health dignity and prevention (PHDP) interventions at HCF.
  • Work collaboratively with the ACT project team in implementing effective data collection, analysis and use to improve patient outcomes in targeted HCF.
  • Participate in both clinical and administrative meetings as needed to promote continuity of care and treatment delivered by the HCF.
  • Plan and provide capacity building for CPS staff and provide supervision, mentorship and coaching within targeted HCF.
  • Liaise with the e MTCT and OVC Advisor and HCF focal persons for e MTCT and OVC to strengthen e MTCT/OVC community interventions including Mother Infant Pair follow up community-facility linkage strategies among others.
  • Provide leadership in expansion of community based interventions in HIV prevention including priority/ key population outreaches, Voluntary Medical Male Circumcision.
  • Provide leadership in operationalization and implementation of the PEPFAR Community Facility linkages Framework at HCF level.
  • Provide technical leadership for all aspects of monitoring and evaluation for community-based interventions for ACT project.

Qualifications/Competencies:

  • A Degree in Nursing, Social Work, Counseling or Community Psychology with 3 years’ experience years working in HIV/AIDS care and treatment setting, Post
  • Graduate training in a related field will be an added advantage.
    Skills and Competencies
  • Experience in HLV/AIDS Counseling
  • Experience in capacity building, such as planning and facilitating trainings and providing technical assistance.
  • Background in community development, management experience in the areas of, reproductive .health, community health, lMCl, MCH and HIV/AIDS is desirable.
  • Excellent written and oral communication skills and report writing skills
  • Strong interpersonal and cross cultural skills
  • Proficiency in Microsoft Office products (Word, Excel, Outlook) .
