JOB DETAILS:

Duties and Responsibilities
• Supporting targeted risk-based HIV testing at different care entry points of the facility. Supporting formation of family Support Groups or Peer support groups (adolescents and discordant couples).
• Supporting health workers to prepare for client visits
• Supporting Linkage and Retention of HIV positive clients identified at all care points including outreaches, TB care point, teenage centers, VCT/PICT to receive HIV care.
• Providing support in the management of non-suppressed clients.
• Following up of clients who miss visits by more than a week using telephone or home visits if they live within your area of residence.
• Linking up with the other community resources to follow up clients e.g. through VHT or community based organizations.
• Supporting documentation and reporting of activities.
• Performing any other duties that may be assigned by the Supervisor.

Key Outputs
• Clients screened and offered HIV testing.
• Lost clients followed up and returned to care.
• Health education talks given.
• Monthly reports submitted.
• Newly identified HIV clients Successfully linked to care.

Training and Qualifications
• ‘O’ Level certificate
• Basic training in HIV/AIDS
• Possess basic Peer Counselling skills
• Fluent in English and local language
• Living with in the health facility catchment
