Applications filled in triplicate on Public Service Forms No.3 (Revised 2008) Should be submitted to the Secretary District Service Commission - Ibanda, P.0. Box 388 Ibadan, not later than 23rd January, 2017

Application Forms should be filled in triplicate and should bear the title of the Post and Reference Number specified against the Vacancy. Applicants should attach Photocopies of Academic Qualifications, CVs, three Passport Photographs (Colored) and any other Relevant Documents.
