General summary:

To provide medical care, treatment and prevention to clients and their care takers (most especially HIV/AIDS clients), and contribute to continuous professional development in line with Good clinical practices

Duties and Responsibilities
• Provide clinical assessment and evaluation of health of clients and provide proper individualized patient centered care and treatment in line with Good clinical practices

Main locations:

1. Prepare and submit periodic activity work plans, monthly timesheets and periodic reports to key stakeholders in line with Health facility targets.
2. Provide appropriate advice, health education and counselling to patients.
3. Monitor implementation of comprehensive HIV care packages at HFs as per the set targets.
4. Conduct pharmacovigilance procedures and monitor disease progression while providing appropriate management including referral for complicated cases to higher levels of management and for community follow up in line with Good clinical practices and National drug authority guidelines
5. Develop patient management plans for TB Clients as per the client’s T.B status and follow them up until they normalize in line with Good clinical practices
6. Perform the roles of a Quality improvement team member and monitor implementation of QI projects for HF in liaison with the HF in charge in line with MoH guidelines for QI
7. Prepare and present periodic findings regarding critical cases n at case conferences, and Health facility continuous medical education sessions as per clinicians’ schedule to promote in continuous professional development.
8. Provide technical support to at least three to four small volume health facilities in his or her catchment area.

Qualifications and job specifications:
1. Diploma in Clinical medicine and community health or equivalent
2. Active registration with the Allied health professionals’ council
3. At least two years of experience in clinic or hospital-based services.
4. Experience in HIV/AIDS and ART management is an added advantage.
5. Willingness to work and live in rural areas is required.

General summary:
The Medical Records Officer shall be responsible for maintenance of up to date Uganda EMR (Open MRS) system with all patients’ records captured. They shall mentor district medical clerks in proper capture and management of HMIS data collected at health facilities and communities. They will provide support to the field office M&E officer in collection, entry, cleaning and analysis of data

Duties and Responsibilities:
Maintain an up to date site Electronic Data base, the Uganda EMR (Open MRS) system:
i. Enter all required records in electronic data base -Uganda EMR (Open MRS)
ii. Carry out data cleaning of all records entered
iii. Extract data from the system according to data requests availed.
iv. Troubleshoot and maintain the electronic systems
v. Support the nearby Health Facilities with Data entry into Uganda EMR as assigned.

Support and mentor the health facility medical records clerks in the following:
i. Registration of all patients
ii. Recording patient visits and other registration data in appropriate HMIS tools
iii. Preparing patient files for the clinic day
iv. Filing all patient files at the end of the clinic and all laboratory test /results.
v. Ensuring that the medical records room is orderly and all files accounted for
vi. Ensuring completeness, consistency and accuracy of data captured in the HMIS
vii. Timely compilation of the routine HMIS reports
viii. Ensuring availability of required HMIS tools i.e. registers, forms, guidelines etc.


Provide assistance to the M&E officer in the following tasks:
i. Perform data abstraction from the HMIS, EMR and other sources for project reporting.
ii. Any other duties as assigned by the supervisor.

Education Requirements:
1. Degree in IT, IS, Statistics, Economics, QE, Population Studies or any data management related field

Required experience and skills:

1. At least two-year experience in data management and analysis
2. Excellent communication and Computer knowledge and skills
3. Knowledge of Health Information Management Systems
4. Accuracy skills
5. Ability for team work

JOB DETAILS:

General summary:
Responsible for collection, validating, entry of data in the respective data bases and maintaining an updated secure filling records system for easy access as well as generating periodic reports.

Duties and Responsibilities:
1. Generate and share lists for ‘every clinic day expected patients’ before the clinic commences, produce and submit a ‘missed visit’ report on every clinic day by 4pm to the community linkage stakeholders like LRAs in line with MoH data management guidelines.
2. Track patients’ files especially ones without clinic exercise books and retrieve any information required by the key stakeholders in line with data management guidelines
3. Generate a monthly report for all patient’s clinic attendances, unscheduled, scheduled, new patients in line with MoH data management guide lines
4. Receive and register newly referred/ linked patients on every clinic day, assign each a distinctive clinic number, and updating appointments in line with clinic flow guidelines and in line with MoH data management guide lines
5. Record post patients’ data from their files into the facility’s electronic data base (Uganda EMR) on a daily basis in line with MoH data management guide lines
6. Collect, clean and routinely validate (weekly, monthly, quarterly and annually) data/information in health facility and from community level in line with IDI and MoH data management guide lines
7. Provide necessary data tools to all health facility staff and provide technical support in data documentation in line with MoH data management guide lines
8. Develop and submit periodic reports to key stakeholders and give timely feedback to the health facility staff in line with IDI and MoH reporting guidelines
9. Develop & maintain proper filing, storage and retrieval systems of patients’ medical records in the health facility in line with MoH data management guide lines
10. Periodically disseminate the updated health facility ‘Performance Monitoring Plan (PMP)’ and quality improvement indicators to track the performance of health facility in line with MoH data management guide lines
11. Any other duties as assigned by the supervisor

Education Requirements:
1. A diploma in Computer Science, Information Management and Data Management

Required experience and skills:
1. Prior work experience in a busy HIV/AIDS setting is an added advantage
2. 1-2 years of related experience
3. Willingness to work and live in rural areas is required
